• Consent record: the consent text you agreed to, the version of that text, your IP address, browser user-agent, and timestamp at the moment of submission.
• Marketing preference: whether you opted in to receive marketing communications (which you may withdraw at any time).
• Attribution data: URL parameters such as utm_source, utm_medium, utm_campaign, utm_term, utm_content, gclid, and fbclid; the page you landed on; and the referring website (if any).
• Submission data: the fields of the form itself (e.g., name, email, phone, message, referral details).

We never send your name, email address, phone number or message to an advertising platform.

Website Chat Assistant

Our website offers a chat assistant powered by artificial intelligence. It is not a person. Messages you type are processed by our AI provider, Anthropic, to generate replies, and we keep chat transcripts so our team can follow up and improve the service. If you send your contact details through the chat, they are handled the same way as our contact form.

Please do not share Social Security numbers, license numbers, financial details, or any patient information in the chat. To ask about or delete a chat transcript, contact us using the details below.
