The Role:
- Location: Westminster, CO 80023; Thornton, CO 80241 (Providers will work 2 days at one location and 3 days at the other; not working at both sites on the same day.)
- Assignment duration: 9/3/2026 - 12/3/2026 (91 days)
- Shift Type: Days - Clinic/Procedures; Mon-Thurs or Mon-Fri: 8a-5p with 1 hour unpaid lunch. Regular Hours: Required.
- Cases: Clinic procedures include colposcopy, IUD insertion, LEEPs, endometrial biopsies, reading ultrasounds. Hospital procedures include hysteroscopy, D&C, hysterectomies. Must be able to perform OB and GYN duties.
- Site Charting System: Epic.
- Credentialing: Timeframe: 60 days standard, 30 days or less for temps Credentialing with other facilities? Yes If so where?: 4075 E 128th Ave, Thorton, CO 80241 Frequency of work at each facility: 2 shifts at one location and 3 shifts at the other. Not working at both sites on the same day. Requirements: Self inquiry NPDB report must be provided and CO license or Active IMLC to release preapplication. Requirements: Preapp and full app must be submitted within 72 hours of receipt of initial app to allow enough time for AD account creation for EMR requirements. Once both are submitted, provider is moved to "applicant in training" which initiates AD Account creation for provider EMR account and can take up to 5 days before release of EMR training requirements. Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? No Emergency privs? Yes, with clean file.
